1. DO NOT submit PROOF OF AGE (POA) to our front desk. You MUST possess POA for each player, at each game, or risk forfeiture. Acceptable items: copy of player card (MSYSA/USYS), copy of birth certificate, passport or ID.
2. Soccer Dome ROSTER FORM (found on the "YOUTH LEAGUES" drop-down menu) MUST be submitted at your first game before any of the players step on the field. We do not accept them electronically. Take a picture for your records before turning it in.
