1. DO NOT submit PROOF OF AGE (POA) to our front desk. You MUST possess POA for each player, at each game, or risk forfeiture. Acceptable items: copy of player card (MSYSA/USYS), ID or copy of birth certificate. Failure leads to inability to play.
2. Soccer Dome ROSTER FORM (found on the "YOUTH LEAGUES" drop-down menu) MUST be submitted 1 week before your 1st scheduled game. Email to youthsoccer@soccerdome.com
